For Referring Practitioners
Referring practitioners can access our eReferral template to refer patients for a number of specialised services at our clinic.
The Melanoma Centre welcomes referrals from GPs and other healthcare professionals for assessment and treatment of a range of skin concerns.
Our referral form is available to download as a PDF or RTF template for Best Practice and other compatible clinical software, making it easy to integrate into your existing referral workflow.
Refer a Patient
Patients can be referred to The Melanoma Centre for:
- Photodynamic Therapy (PDT), including Red-light PDT and Daylight PDT
- Erbium laser
- Full skin checks
- Spot checks
- Biopsy
- Excision
The referral form also allows you to provide relevant diagnosis, previous treatments and response, medical history or immunosuppression, current medicines and allergies, and any photosensitivity history or concerns. MLC referral

Ongoing Patient Care
Where appropriate, we can assess and treat the referred concern and provide relevant correspondence back to the referring practitioner to support the patient’s ongoing care.
